How do I start services with True Heart Care Services?

Getting started with True Heart Care Services is a straightforward and supportive process. The first step is to reach out for a chat where we have the opportunity to get to know each other and understand what you’re looking for. We’ll take the time to learn about your goals, current support needs, and how you prefer things to be done. 

From there, we’ll walk you through the next steps, including preparing a tailored support plan that suits your NDIS goals and funding. If you’re plan-managed, we’ll also communicate directly with your plan manager to make things as seamless as possible. For self-managed participants, we can provide service agreements and invoices in a way that keeps everything clear and easy to manage.

At the moment, True Heart Care Services is not a registered NDIS provider as we are in the middle of this process, but that doesn’t limit the quality or range of support we offer. We currently work with participants who are either self-managed or plan-managed, which means you’re still able to access our full range of supports if your plan falls under one of those categories. 

Our team brings strong clinical expertise and years of experience across disability, mental health, and youth services, and we follow the same professional standards expected of registered providers. While we work towards full registration in the future, we remain focused on delivering safe, respectful, and individualised care. Yes, we can. True Heart Care Services offers support across both core and capacity building categories, depending on your goals and what’s included in your plan. Core supports can include things like help with daily personal activities, household tasks, transport, and community participation. Capacity building supports are more about helping you build independence, confidence, and life skills, and we can help there too. 

Whether you’re working towards getting out more in the community, learning new routines at home, or improving your confidence with day-to-day activities, we’ll tailor our support to suit. We work closely with each participant to make sure our services align with your plan and priorities, and we’re happy to chat through what your funding allows and how we can support you best.

Yes, we do. We understand that not all support fits neatly into standard business hours, and we offer flexible options to help meet your needs, including after-hours, evening, and weekend support. Whether you need help at home in the evenings, transport on weekends, or support to attend community events or appointments outside regular hours, we can usually accommodate it. 

Our team will work with you to understand your routine and preferences so we can provide support at the times that suit you best. We know that flexibility matters when it comes to providing the right care, and we’re committed to working around your schedule wherever possible. If you have a particular time in mind, just let us know and we’ll do our best to make it happen.

True Heart Care Services is based in Melbourne, and we currently support participants across a range of suburbs and surrounding areas. We aim to be flexible and responsive, so if you’re unsure whether we can come to you, just reach out, we’re always happy to chat through options.
